Differences between infertility treatments, IUI, IVF and microinjection

The difference between IVF and ICSI microinjection is how the Oocyte is fertilized. In IVF, the sperm is exposed to the Oocyte to enter the Oocyte itself, while in ICSI, the sperm is inserted into the Oocyte with a special needle.

 

ICSI microinjection such as IVF and IUI is one of the assisted reproductive techniques (ART). Unlike IUI, in which male sperm enter the fallopian tube through a fallopian tube, in IVF and ICSI, the female's Oocyte combines with the male sperm outside the body, in vitro, and after fertilization, the resulting embryo enters the uterus. In general, the steps of performing ICSI microinjection operation can be divided into five steps:

Step 1: Stimulate ovulation

Step 2: Collect the Oocyte

Step 3: Prepare the sperm

Step 4: Fertilization and fetal growth in the laboratory

Step 5: Transfer the fetus to the uterus.

Step 6: Pregnancy test.

 

Ovulation stimulation:

By prescribing ovulation-stimulating drugs to a patient's ovaries, large numbers of Oocyte-containing follicles grow. However, with the administration of fertility drugs, a larger number of ovarian follicles grow and reach final maturity, and the ground is prepared for the collection of more Ovum cells. As the number of collected Ovum cells increases, the number of resulting embryos increases and ultimately the chance of fertility increases. Ovulation-stimulating drugs are given to patients in different protocols, which vary according to the patient's Oocyte and condition.

 

These drugs may be used alone or in combination. Ovulation stimulants are often given over a period of 10 to 14 days. Ovulation stimulants are usually started from the first to the third day of the menstrual cycle according to the treatment plan, and from the sixth day, mature follicles are identified using serial ultrasound and daily blood tests.

Finally, HCG is injected to fully mature the Oocyte and release it. Ovulation will occur approximately 36-38 hours after HCG injection. This allows the IVF team to determine the right time to ovulate (puncture).

 

Ovum collection (puncture operation):

The next step after taking ovulation-stimulating drugs is to take an Oocyte or (puncture operation). Ovulation from the vaginal tract by perforating the follicles, their contents, which are Ovum cells and follicular fluid, are inserted into the ovary through a long needle and the mature Ovum cells are collected by a suction pump.

In some cases, the Oocyte is extracted by laparoscopic surgery. Laparoscopy is a surgical procedure that usually requires general anesthesia. In this procedure, the doctor uses a laparoscope to view the ovaries and fallopian tubes, and drains the follicles directly from the ovary using a suction device.

 

Sperm preparation:

On the morning of Oocyte collection, semen is prepared from the patient's spouse, and after washing and preparation, healthy and highly active sperm are removed.

 

Fertilization and fetal growth in vitro:

In the ICSI microinjection method, first the cells around the Oocyte are separated with the help of an enzyme, then the sperm is injected into the Oocyte with a special needle with the help of very advanced microscopes.

A fertilized Oocyte is called an embryo. The unicellular embryo begins to divide and a multicellular embryo develops. If fertilized, the embryos are incubated for another 24 hours to reach the four- to eight-cell stage and ready to be transferred to the mother's uterus. Embryo formation and their number and quality depend on the number and quality of Ovum cells and sperm.

 

Fetal transfer into the uterus:

Embryo transfer is also performed without anesthesia. At this stage, the embryos are placed in a thin tubular device called a catheter and transferred from the vaginal tract to the uterus. The person is usually discharged from medical centers two hours after the transfer of the fetus.

In special cases where it is not possible to transfer the fetus vaginally or at the discretion of the doctor, the transfer of the fetus may be done with the help of laparoscopy, which is called ZIFT.

In a woman between the ages of 20 and 29, with three transferred embryos, the chance of having multiple pregnancies is 46 percent.
